At Lakeview Recovery Community, we understand the importance of a supportive and structured admissions process. 

Our aim is to ensure a smooth transition into our recovery community. Whether you are seeking help for yourself or referring someone, we strive to provide a seamless experience from the first contact to admission.

Eligibility Criteria

To be eligible for admission to Lakeview Recovery Community, applicants must meet the following criteria:

Age: Must be 18 years or older.

Gender: This is a male-only program.

Residency: Must be a resident of Alberta.

Costs: The program, lodging, and food is all covered by the Government of Alberta.  Additional costs may include canteen items, medication purchases, clothing purchases (some donations are available). 

Step 1

Initial Screening

Begin by filling out our online admissions screening form. This initial form will help us determine if you meet the basic eligibility criteria for our program

Step 2

Detailed Admission Assessment

If you meet the initial eligibility criteria, we will email you a detailed admission assessment form. This form requires more comprehensive information to better understand your specific needs and recovery goals.

Step 3

Admission Review

Upon receiving your completed detailed admission assessment form, our admissions team will conduct a thorough review to make an informed decision regarding your application. If accepted, you'll receive an admissions date.

Step 4

Pre-Admission Engagement

Along with regular weekly check-ins with our Lakeview admissions team, you will be assigned a virtual recovery coach through Bowline Health. This coach will engage with you daily to help prepare you for the transformative journey ahead at Lakeview. Recovery coaching is a confidential service provided at no additional cost.

Restricted Medications

The following medications are considered restricted: 
(Note: this list is not exhaustive and other medications may be subject to restriction)

  • Codeine & Codeine containing products (e.g. Tylenol #3)
  • Fentanyl
  • Hydromorphone (Dilaudid)
  • Oxycodone (Percocet, OxyNeo)
  • Meperidine (Demerol)
  • Tapentadol (Nucynta)
  • Tramadol (Zytram, Ralivia, Tridural) (Will be considered on case-by-case bases)
  • Pentazocine (Talwin)
  • Propoxyphene (Darvon)

If you are currently on a restricted medication:

Never stop or change your controlled medication dosing without the direct supervision of a medical practitioner.

OR

Consider requesting an alternative medication that is not on the restricted medication list from a medical prescriber.

Referrals

For Healthcare Professionals, Community Partners, and Family Members

Please start by filling out our online admissions application screening form. This form is the first step in the process for both self-referrals and professional referrals.

For healthcare professionals and community partners, it's important to enter your professional details and confirm that you have obtained consent from the individual to proceed with the application. This ensures that all parties are informed and agreeable to the referral process, maintaining compliance with privacy and ethical standards.

For family members, this form serves as a direct way to initiate the admissions process for your loved one. We understand the critical role that family support plays in the recovery journey and are here to assist you in getting the help your loved one needs.
